Care Management Services

Who Are We?

Care Connection provides a centralized service that helps guide seniors and their caregivers in making decisions about care and living needs. Our service includes information and referral, care coordination, and care Senior couplemanagement. We serve seniors 50 years and older in Greenville, Spartanburg, Cherokee and Union Counties. Services are affordable and are customized to an individual’s needs and range from $5.00/hour to $90.00/hour depending upon a person’s monthly income. Those with incomes over $50,000 per year are served by our Connection One Life Management Services.

Care Management

  • On-going services to ensure safety and quality of life
  • Our Care Manager will visit the individual and Mother and Daughtertake the following steps:

1. Complete an ASSESSMENT. The Care Manager will get a thorough and complete sense of the individual’s health and overall situation, their goals and wishes, and any concerns the family may have.

2. Complete a CARE PLAN. The care plan includes a summary of the concerns and strengths and recommendations for services or care the individual may need to remain independent. The care plan is reviewed with the individual and their family.

3. Based upon the individual’s needs and desires, the Care Manager can then ARRANGE SERVICES.

4. The final task for a Care Manager is FOLLOW-UP and MONITORING. Care managers visit the individual regularly to see how they are doing, ensure that services are being delivered and make any adjustments as the individual’s needs change.

*There is no charge for initial telephone consultation and screening. However, initial assessment and follow-up visits are billed according to fee schedule. We do not receive payments from Medicaid, Medicare or insurance companies.
