Here is some of what we can do:

  • Comprehensive, extensive on-site assessments of an individual’s situation.
  • Create personalized plans that focus on an individual’s health, home and life choices.
  • Work closely with a network of professionals that may include physicians, attorneys, financial planners, accountants and trust officers to effectively manage all aspects of an individual’s life.
  • Avert crisis situations relating to an individual’s health and care needs.
  • Arrange services such as transportation, in-home sitter care, medical equipment, and medication monitoring.
  • Budgeting and bill paying.
  • Act as a liaison with healthcare and service providers.
  • Act as a “surrogate” family member especially for those individuals who have no immediate family or whose family lives out-of-town.
  • Coordinate and accompany individuals to doctor’s appointments.
  • Assist with applying for benefits such as Veteran’s, Medicare, Social Security, etc.
  • Help with decisions about assisted living or nursing home placement.
  • Long-term monitoring in a home environment or care setting.
